Infectious Diseases Institute (IDI) at Makerere University Medical School in Kampala Uganda (http://www.idi-makerere.com/ ), is an outpatient clinic serving People Living with HIV (PLHIV), training healthcare workers and to conducting research relevant to improving the outcome HIV AIDS.

As part of a research program IDI required a custom software package designed for the management of ARV patients and ARV therapy in an African setting. The software requirements included a robust database to hold significant quantities of detailed medical information; mechanisms to ensure high data quality; and a scalable architecture for easy addition of new medical forms.

SOLUTION

A software application was built to accommodate the data from baseline and follow up medical forms for children and adults. Because the amount of data per patient was quite large, special care was take with the user interface to segment the data input screens and encode business rules to aid in the entry of correct and consistent data. Simple reports calculating population statistics was provided.

SPECIALIZATION

Because the data was going to be used for research purposes, it was imperative that the data is clean and consistent and the data set fully populated. Existing systems did not have this kind of data quality emphasis. The system captures over 450 pieces of data per patient per medical form. Field level validation is used extensively as well as conditional user input elements. Age and gender specific rules were implemented to hide/unhide appropriate questions and field combinations driven by the patient’s profile. Workflow was implemented so that quality assurance specialists can review the data before it is available for research purposes.

IMPACT

IDI was able to collect clean data for research purposes and with the quality control needed to assure that the data met the standards of the donor. This work set up IDI’s programming environment and set up processes for further development projects by IDI’s software development team.

A global reinsurance company wanted the ability to sell individual risk reinsurance over the web and needed a way to reduce transactional costs. Multilingual content and transaction forms were required.

SOLUTION

Build a fully interactive website that allowed a client to purchase reinsurance online. Provide an administration portal for the company employees to monitor workflow and measure throughput.

Developing an online portal that displayed the client’s entire portfolio of reinsurance books required integration with multiple backend databases including mainframe data. This integration required detailed data analysis, mapping, export, loading and transformation into a centrally managed database.

IMPACT

This project revolutionized how the reinsurer sold reinsurance. The company’s arability to renew business was made significantly easier because the customer could see all of the business booked with the reinsurer. The data consolidation opened up new opportunities to analyze and use the data to optimize operations.

A healthcare software company needed the ability to aggregate, categorize and access multiple sources of third party content and access the content through its main patient facing application.

SOLUTION

A stand-alone tool was developed that allowed medical librarians and nurses to categorize and aggregate all of the sources of the third party content. First, all of the third party content, including web pages, databases and files, was crawled and indexed using search engine technology. Once indexed, the content was categorized. The tool also handled medical news derived from  three news sources.

Substantial planning and design was required to create a tool that appeared simple yet was flexible enough to provide users with the ability to use any taxonomy and manipulate the categories as they wanted.

The Nigerian Ministry of Health wanted to collect patient level data and see trends for key public diseases across the country so that they can set healthcare policies and allocate resources effectively

SOLUTION

In an initiative funded by the US Center for Disease Control, the Nigerian MOH in collaboration with Futures Group and Project Balance developed a national healthcare data mart storing HIV care and treatment indicators based on the national HIV Care Card. The data mart is estimated to hold over 2.5 M records for more than 300,000 patients. There is a business rule driven extract, transformation and loading process tailored to the 7 different electronic medical records systems used in country feeding a star schema data mart. There are standard data visualizations and key reports; users can drill into the visualization down to the facility level.

The export, transformation and loading (ETL) were adapted to each of the implementing partner’s electronic medical record systems. Complex business rules were applied to the data to check available data and optimize the data set.  Detailed quality testing was done to assure the business rules are accurate.

EXPECTED IMPACT

This will be the first time that data will be aggregated from facilities with EMRs across the country. Trends in outcomes at a regional and local levels and implementing partner is expected to help the ministry of health allocate funding and human resources more effectively. The ability for implementing partners in the country to consistently pull high quality data is expected to expose gaps in data extract processes and data quality that the Ministry of Health will need to address for the sustainability of the data warehouse.

An electronic medical record system was needed in developing countries to support the President’s Emergency Fund for AIDS Relief (PEPFAR) initiative which commenced in February 2004. In the years since the program started a number of electronic medical records systems were built and still continue to be actively used. IQCare is one such system, currently installed in over 400 facilities in Kenya one of which is Kenyatta National Hospital, the largest hospital in eastern Africa, Nigeria, Uganda and Zimbabwe. As needs expanded, so did the software; starting from a single focus of collecting HIV care and treatment data to the current version which is highly configurable and is being used in many different types of clinical departments.  For more information about the product, see http://www.iqstrategy.net/products/iqcare/ .

SOLUTION

Futures Group International in collaboration with Project Balance built an open source .Net, C#, web-based application to fulfill the data capture and monitoring and evaluation needs of the program. With an emphasis on data demand and use, the collection of data in static and dynamic (form builder forms) a user friendly web interface was developed. Because IQCare is installed in all sizes and types of clinics, most features are configurable to meet the needs of a wide range of clinics. Special focus was put onto capturing clean and accurate data and field level validate and cross form business rules are programmed into every form, both static and dynamic.

IMPACT

As IQCare has matured as a product, features such paperless data entry by clinicians and supply chain management has had a significant positive effect on patient management and facility operations. The product’s form builder has allowed facilities to expand the use of IQCare into many different departments or service areas. Clinicians can see data for patients receiving treatment in multiple departments allowing them to have a comprehensive view of the patient’s clinical status. This kind of information combined with informational alerts help clinicians make more informed treatment decisions to provide better care.
